Job summary

Home Health Care, Inc. is seeking an experienced RN Case Manager for a home-based role serving the St. Paul area. You will manage caseloads, perform OASIS assessments, and deliver skilled nursing care with a focus on patient education and safety.

This full-time position offers a Monday–Friday schedule, territory coverage within ~35 miles, and a sign-on bonus. You will work with an interdisciplinary team to optimize patient outcomes and independence at home.

Qualifications

Job Title: Registered Nurse (RN) Case Manager – Home Health | Where Your Experience Makes the Difference

Territory: St. Paul + approximately 35 miles from your home for on-call and weekend coverage

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week
