Home Health Care, Inc. is seeking a Registered Nurse (RN) Case Manager to lead one-on-one home health visits across Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.

This full-time role offers predictable weekday hours, OASIS assessments, and tailored plans of care designed to help patients stay safe and independent at home. Responsabilidades

• Manage a home health caseload across Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
• Complete OASIS assessments and individualized plans of care.
• Provide skilled nursing visits, assessments, and patient education.
• Coordinate care with physicians and the interdisciplinary team.
• Help patients remain safe and independent in their homes.
• Maintain accurate and timely clinical documentation.

Home Health RN Case Manager | Community-Based Nursing | $6,000 Bonus

Job Title: Registered Nurse (RN) Case Manager – Home Health | Reclaim Your Work-Life Balance

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses
