Job summary

Maricopa County Department of Public Health is seeking an RN Case Manager to coordinate Early Intervention and Transitional Case Management for HIV-positive clients released from jail. You will provide counseling, health literacy, and linkage to community-based services to ensure continuity of care.

The role involves discharge planning, collaboration with CHS and jail staff, and HIPAA-compliant documentation within CAREWare and surveillance systems.

Qualifications

• Two years of Registered Nurse experience.
• Valid unrestricted license as a Registered Nurse issued by the Arizona Board of Nursing (AZBN).
• Bachelor’s degree in nursing from an accredited institution.
• An equivalent combination of post‑secondary education and/or nursing experience may substitute for the minimum qualification on a year‑for‑year basis.
• Bilingual skills in English/Spanish.
• Experience with interviewing techniques, conducting complex nursing assessments, and providing client care.
• Case management experience working with individuals with communicable diseases.
• Valid Arizona driver’s license required upon hire.
• Must secure and maintain an Arizona Department of Public Safety Level 1 Fingerprint Clearance Card and complete a background check.

Responsibilities

• Identifies clients in the jail system who may qualify for transitional case management services.
• Reviews client’s release status using jail data systems and medical records.
• Provides transitional case management, including discharge planning and linkage to community HIV care services.
• Completes HIV nursing assessments with medical, dental, substance abuse, mental health, and social histories.
• Develops individualized discharge plans and initiates referrals to community outreach and case management agencies.
• Provides counseling for incarcerated HIV clients on chronic condition management and treatment adherence.
