Job summary

Maricopa County Department of Public Health is hiring an RN Case Manager to coordinate Early Intervention and Transitional Case Management services for HIV-positive clients released from the county jail. You will conduct discharge planning, health literacy, and linkage to community-based HIV care, while providing counseling and care coordination within a correctional setting.

Qualifications

• Two years of Registered Nurse experience required.
• Valid unrestricted RN license by the Arizona Board of Nursing (AZBN).
• Bachelor’s degree in nursing from an accredited institution.
• Equivalent combination of education and/or nursing experience may substitute for the minimum qualification.

Responsibilities

• Coordinate transitional case management services for HIV-positive clients released from jail to the community.
• Provide counseling, health literacy, and linkage to HIV care services.
• Discharge planning and referrals to community resources; document in medical records.
