Job summary

TemPositions Group of Companies is seeking a Nurse Case Manager to coordinate health care services for adults with disabilities in residential and day programs in the New York metropolitan area.

You will monitor medications, consult with physicians, supervise staff, and ensure timely medical follow-up while maintaining confidential records and collaborating with the interdisciplinary team to achieve positive health outcomes.

Qualifications

• 1-2 years RN experience preferred.
• Valid NYS RN license in good standing required.
• Associates Degree in Nursing required.
• Knowledge of OPWDD regulations preferred.
• BLS, CPR certified; SCIP-R certification required.
• Ability to lift and move people up to 20 lb safely.
• Strong organizational and interpersonal skills; maintain confidentiality.

Responsibilities

• Coordinate delivery of health care services for adults with disabilities in residential/day programs.
• Oversee health issues for people with autism in Residential or day programs.
• Monitor medication and medical records for individuals receiving services.
• Consult with MDs to ensure optimal medical treatments are provided.
• Supervise DSP staff administering medication; ensure timely follow-up.
• Ensure individuals receive prescribed medications, treatments and nutrition as ordered.
• Accompany or transport individuals to appointments as needed.
• Maintain accurate mandated medical records.
• Attend internal team meetings; work with interdisciplinary team for positive health outcomes.
• Provide guidance and training to direct care staff on health care delivery.
