Job summary
A healthcare services provider in New York is seeking a Nurse Case Manager to coordinate health care for adults with disabilities. The ideal candidate will have an Associates Degree in Nursing, valid NYS RN license, and strong organizational and communication skills. Responsibilities include overseeing medication management and consulting with healthcare providers. Qualifications
- 1-2 years LPN experience highly preferred.
- Valid NYS LPN license in good standing required.
- Knowledge of OPWDD regulations is highly preferred.
- BLS, CPR certified; SCIP-R certification required.
- Ability to safely assist lifting individuals of various weights & 20 lb. items required.
- Must be able to maintain Individual/family confidentiality.
Responsibilities
- Coordinate the delivery of health care services for adults with disabilities.
- Monitor medication and medical records for people receiving services.
- Consult with MD's to ensure optimal medical treatments.
- Supervise staff administration of medication.
