Aviator Home Health is seeking a dedicated Registered Nurse (RN) Case Manager in Texas to join a clinical team focused on high-quality home health care. You will perform comprehensive assessments and develop care plans while coordinating with physicians and staff to optimize patient outcomes.

The role requires an active Texas RN license and at least 1 year of RN experience; home health and OASIS familiarity are preferred. We offer full-time work with robust benefits and supportive management.

Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA)Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service.
