Job summary

Aviator Home Health is seeking a Registered Nurse to join our clinical team delivering skilled home health care in the Dallas–Fort Worth area. You will perform comprehensive nursing assessments, develop and revise plans of care, and supervise staff while ensuring high-quality patient outcomes.

The ideal candidate has Texas RN licensure, at least 1 year of RN experience, and familiarity with OASIS processes. Experience in adult home health and HomeCare HomeBase is preferred.

Qualifications

• Must hold an active Texas RN license.
• One year of RN experience in home health is preferred.
• OASIS knowledge is preferred.
• Strong communication and patient-care skills are essential.

Responsibilities

• Perform initial and ongoing comprehensive assessments and OASIS as appropriate.
• Develop, initiate, and revise the plan of care for patients.
• Provide skilled nursing care with infection control measures.
• Coordinate patient care, referrals, and changes with the physician and team.
• Complete timely and accurate documentation.
• Supervise and direct HHA/LVN staff; participate in on-call coverage.

Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA)Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service.
