Job summary

Aviator Home Health is seeking a Registered Nurse to join our clinical team delivering skilled home health care in the Dallas–Fort Worth area. You will perform comprehensive nursing assessments, develop and revise plans of care, and supervise staff while ensuring high-quality patient outcomes.

The ideal candidate has Texas RN licensure, at least 1 year of RN experience, and familiarity with OASIS processes. Experience in adult home health and HomeCare HomeBase is preferred.

Qualifications

  • Must hold an active Texas RN license.
  • One year of RN experience in home health is preferred.
  • OASIS knowledge is preferred.
  • Strong communication and patient-care skills are essential.

Responsibilities

  • Perform initial and ongoing comprehensive assessments and OASIS as appropriate.
  • Develop, initiate, and revise the plan of care for patients.
  • Provide skilled nursing care with infection control measures.
  • Coordinate patient care, referrals, and changes with the physician and team.
  • Complete timely and accurate documentation.
  • Supervise and direct HHA/LVN staff; participate in on-call coverage.

With Aviator, you're not just a clinician out in the field, you'll be part of a CLINICAL TEAM that works together for the betterment of our patients with unparalleled support from our office and management staff.Essential Functions:Initial and ongoing comprehensive assessments of home health patient’s needs, including OASIS assessments at appropriate time points.Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.Provide those services and/or treatments requiring substantial and specialized nursing skills. Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA)Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service. Monitor assigned cases to ensure compliance with requirements of third party payor.Provide effective communication/counseling to patient/family, team members, and other health care professionals in meeting nursing and other needs.Complete appropriate documentation in a timely manner.Supervise and provide clinical direction to HHA/LVN providing services.Demonstrate commitment, professional growth by participating in in-service programs and competency.Promote Agency philosophy and administrative policies.Perform on-call responsibilities and provide on-call service to patients/families as assigned.A qualified Registered Nurse will possess:Good organizational and time management skillsExcellent verbal and written communication skillsStrong interpersonal skillsAbility to multi-task and accomplish multiple goals simultaneouslyProfessional demeanor and appearanceExcellent customer relations skillsMeets health requirements of TexasCompetency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice ActExperience Needed for the Registered NurseMust be a registered nurse (RN) licensed in Texas (Required)One year of RN experience (Required)Adult home health experience (Preferred)HomeCare HomeBase experience (Preferred)OASIS experience (Preferred)Aviator Home Health is one of the fastest growing companies in the DFW.
