Job summary

Aviator Home Health is seeking a dedicated Registered Nurse (RN) Case Manager in Texas to join a clinical team focused on high-quality home health care. You will perform comprehensive assessments and develop care plans while coordinating with physicians and staff to optimize patient outcomes.

The role requires an active Texas RN license and at least 1 year of RN experience; home health and OASIS familiarity are preferred. We offer full-time work with robust benefits and supportive management.

Qualifications

• Must be a licensed RN in Texas.
• One year of RN experience required.
• Home health experience preferred.
• OASIS experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of home health patients.
• Develop, initiate, revise the plan of care and provide skilled nursing services.
• Coordinate care, referrals, and communicate changes to the physician and team.
• Supervise HHA/LVN and ensure timely documentation.
• On-call responsibilities as assigned.
