D&S Community Services, part of the Sevita family, seeks a Registered Nurse (RN) for IDD Services in North Austin, TX. The role emphasizes nursing case management, healthcare coordination, and delegation rather than hands-on nursing, with a typical Monday–Friday schedule and travel within the North Austin area.

You will manage an assigned caseload, support direct care staff, coordinate physician visits, monitor medications, and document care.

Qualificações

• Graduate of an accredited RN program with Texas RN license.
• Current CPR/First Aid certification.
• Experience in healthcare coordination or IDD settings preferred but not required.

Responsabilidades

• Conduct nursing assessments, quarterly reviews, and monthly visits.
• Develop and monitor individualized healthcare plans and respond to condition changes.
• Coordinate physician appointments and follow-up care.
