Job summary

Sevita, through D&S Community Services in San Angelo, TX, seeks a Registered Nurse to deliver IDD services. The role includes assessments, care planning, and medical oversight across multiple locations with travel required.

The position follows a Monday–Friday 8:00am–5:00pm schedule with on-call every 5 weeks, and offers a comprehensive benefits package including medical, dental, vision, life and disability coverage, 401(k) with company match, and wellness resources.

Qualifications

• Graduate of an accredited RN program.
• Current state RN license.
• Current CPR/First Aid certification.

Responsibilities

• Conduct assessments, implement health care plans, and respond to changes of condition.
• Communicate with physician and external medical centers to ensure consistency of essential medical information.
• Provide technical assistance, medical oversight, and training of staff.
• Provide input regarding health care services to be included in each individual’s service plan.
• Monitor and ensure physicians orders are documented and carried out as prescribed.
• Travel between multiple locations to provide support.
