Job summary

DayToDay Health is seeking a Registered Nurse Case Manager to develop and implement personalized home care plans for IDD services, ensuring compliance with medical standards and agency policies.

The role requires coordinating intake assessments, setting measurable goals, reviewing medical data for progress, and training aides while supervising LPNs and maintaining clear communication among clients, families, and care teams.

Qualifications

• Bachelor’s degree in nursing required.
• Valid RN license in the state.
• At least 2 years of nursing experience in case management or IDD services.
• Strong assessment, communication, and supervisory abilities.
• Ability to travel as needed; physically able to perform duties.

Responsibilities

• Conduct client intake assessments.
• Develop measurable, person-centered care goals.
• Review medical data to track progress and adjust plans.
• Address client or staff concerns promptly.
• Train aides to meet client needs and safety standards.
• Supervise LPNs and coordinate nursing staff.
• Perform periodic evaluations of care plans and outcomes.
• Facilitate effective communication among clients, families, and care teams.
