Job description

Job Title: RN Case Manager | Home Health Float | Go Where You're Needed Most

Territory: Greater Twin Cities Metro | Rotating Territory Assignments Across Our Service Area

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $95,000 – $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses

Some Nurses Build Their Career in One Territory. You'll Build Yours Across All of Them.

When one of our territories needs additional clinical support, we need an experienced nurse who can step in and take ownership.

That's the Float RN Case Manager.

Rather than permanently managing patients in one geographic area, you'll rotate throughout different portions of our service area based on patient census, staffing, and clinical needs .

You'll typically spend approximately three months supporting an area , giving you time to learn the territory, manage your patients, work alongside the local clinical team, and become a dependable resource.

Then, when the need changes, you'll take that experience somewhere new.

This isn't random floating from one end of the metro to another every morning.

It's structured flexibility—and an opportunity to become one of the most versatile nurses on our team.

Be the RN We Can Send Anywhere

There's a certain kind of nurse every clinical team wants.

The one who walks into a busy situation and doesn't get overwhelmed.

The one who learns quickly.

The one who communicates.

The one who takes ownership.

The one who doesn't need six months to become effective.

That's who we're looking for.

As a Float RN Case Manager , you'll develop familiarity with multiple territories, patient populations, clinicians, and communities throughout our organization.

Over time, you'll become the person we can confidently send into a territory knowing:

You'll figure it out. You'll support the team. And most importantly, you'll take great care of the patients.

How the Float Position Works

Your territory will change based on where additional Case Management support is needed.

Rather than changing every few days, assignments will generally last approximately three months at a time .

During each rotation, you'll:

  • Become familiar with that portion of our service area
  • Manage an assigned home health caseload
  • Build relationships with patients and caregivers
  • Work closely with the interdisciplinary team supporting that area
  • Provide continuity throughout your patients' episodes of care
  • Help stabilize coverage and ensure patients continue receiving timely care

When organizational needs shift, you'll transition to another territory and become part of that team.

New area. New patients. New challenges. Same standard of nursing.

Why the Right Nurse Will Love This Role

  • $95,000–$130,000 annual compensation plus incentive visit opportunities
  • $6,000 in first-year bonuses
  • Structured territory rotations instead of one permanent geographic assignment
  • Rotations typically lasting approximately three months
  • Opportunity to work with multiple clinical teams
  • Exposure to different communities and patient populations
  • Build a broader understanding of home health operations
  • Meaningful 1:1 patient care
  • Independence in the field with experienced clinical support
  • Cell phone reimbursement for eligible clinicians
  • Free access to our MedBridge Education Hub
  • Licensure reimbursement at renewal
  • Comprehensive medical, dental, vision, PTO, paid holidays, and 401(k)

You're Not Just Making Visits. You're Taking Ownership of the Case.

Floating doesn't change what it means to be a Case Manager.

When you're assigned a patient, you're responsible for understanding the whole clinical picture.

You'll assess where they are today.

Understand why they entered home health.

Develop and manage their plan of care.

Coordinate with physicians and therapists.

Educate patients and caregivers.

Recognize when something changes.

And help move that patient toward their goals.

Your ZIP code may eventually change.

Your responsibility to your patients doesn't.

What You'll Do

  • Manage an assigned home health caseload within your current coverage territory
  • Rotate throughout different portions of our service area based on clinical and staffing needs
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Develop and update individualized plans of care
  • Perform comprehensive nursing assessments and skilled visits
  • Provide wound care, medication management, patient education, and other skilled nursing services
  • Monitor patient progress and recognize changes in condition
  • Communicate with physicians regarding clinical concerns
  • Coordinate with PT, OT, SLP, social work, and other interdisciplinary team members
  • Help patients avoid unnecessary hospitalizations and remain safely at home
  • Maintain timely and accurate clinical documentation in Homecare Homebase (HCHB)

This Position Isn't for the Nurse Who Needs Everything to Stay the Same.

We're looking for someone who sees change and thinks:

I can handle that.

Someone who can learn a new area.

Meet a new team.

Take over a caseload.

Build trust with new patients.

Understand what needs attention.

And become productive without needing someone to constantly point them in the right direction.

  • Active Minnesota Registered Nurse (RN) license
  • Strong comprehensive assessment and clinical decision-making skills
  • Home health and OASIS experience strongly preferred
  • Confidence managing an independent patient caseload
  • Strong organization and time-management abilities
  • Excellent patient, caregiver, physician, and team communication
  • Flexibility to transition between territories as organizational needs change
  • Reliable transportation and willingness to travel throughout our service area

Why Home Health Care, Inc.?

For more than 30 years , Home Health Care, Inc. has provided skilled home health services throughout Minnesota.

Our size and service area create opportunities for nurses to build meaningful careers in the field—but they also mean patient needs aren't always evenly distributed.

That's why this position matters.

You're the flexibility behind our ability to keep providing great care.

When one territory grows, you can help.

When another team needs support, you can step in.

When patients need an experienced Case Manager, you're ready.

And after doing that across multiple areas, you'll understand our patients, our teams, and our organization in a way very few nurses do.

One RN. Multiple Territories. Organization-Wide Impact.

Learn the territory.

Own the caseload.

Support the team.

Take care of your patients.

Then bring everything you've learned to the next area that needs you.

If you're an experienced RN who enjoys variety, autonomy, problem-solving, and being the person others can depend on , we'd love to connect.

Don't just become an expert in one territory. Become the RN we can trust in any of them.
