Job summary

Adara Home Health, Inc. seeks an RN Case Manager to provide direct client care and oversee home health services in the community. You will collaborate with clients, families, and the health care team to develop goal-centered care plans considering quality and financial aspects of treatment.

The role reports to the Clinical Supervisor and requires Minnesota RN license with 2+ years in health care. Travel to client homes for 1:1 care is required; strong communication and documentation are

Qualifications

• Two years of experience in a health care facility or home care.
• Ability to work independently and manage priorities.
• Excellent written and oral communication skills; teach and train others.
• Current CPR certification is highly recommended.

Responsibilities

• Develop and update the home health plan of care at least every 60 days.
• Provide direct client care to diverse ages and conditions.
• Coordinate care with family, caregivers and health team.
• Provide clinical oversight and leadership to staff.
• Ensure documentation and payer/billing compliance.
