Job summary

Adara Home Health in Minnesota seeks a Clinical Admissions Nurse to ensure client care aligns with Policies and Procedures, maintain strong relationships with referral sources, and report to the Clinical Supervisor. This full-time role requires travel to clients' homes for 1–3 hours per client, 1:1 client care, and participation in an on-call rotation.

Base salary range is $70,000–$95,000 per year with benefits including medical/vision/life, 401(k) with full vesting, tuition reimbursement, paid

Qualifications

• RN licensed in Minnesota with unrestricted status.
• Minimum of 2 years experience in a health care facility or home care.
• Strong organizational skills and the ability to work independently.
• Excellent written and oral communication skills; interpersonal communication and teaching skills.
• Current CPR certification recommended.
• Knowledge of Medicare certification and OASIS documentation.
• Access to dependable vehicle or public transportation to travel to multiple stops.
• Valid driver’s license and proof of car insurance for business travel.
• Background checks including MDH and OIG requirements.

Responsibilities

• Performs initial and periodic assessment of clients and develops care plans.
• Records nursing diagnoses, measurable outcomes, interventions and nurses notes.
• Provides total client care including medications and treatments as ordered by the physician.
• Coordinates client care with family, referrals and other disciplines.
• Maintains confidentiality and infection control standards.
• Participates in after-hours on-call process.
• Teaches and evaluates delegated tasks; provides orientation to employees.
