Job summary

Hospice of the Valley in Phoenix (AZ) is seeking Registered Nurses to join as RN case managers providing and coordinating hospice care in patients' homes or care facilities.

As part of an interdisciplinary team, you will educate patients and families, manage symptoms, and ensure compassionate, high-quality care. This full-time role follows a day shift (8a-5p) with 40 hours per week in a not-for-profit organization.

Qualifications

• Graduate of accredited school of nursing.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution driven, creative and resourceful problem-solving skills.
• Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
