Job summary

Hospice of the Valley is seeking an RN Case Manager for the Arizona Supportive Care (AZSC) team in the Central Phoenix territory. The role delivers one-to-one, compassionate care primarily in the patient’s home or care facility, coordinating with physicians to manage daily living and disease management needs.

The candidate will develop therapeutic relationships, assess and plan care, and document meticulously while maintaining high professional standards.

Qualifications

• Graduate of accredited school of nursing.
• OASIS C experience.
• Minimum one year of recent medical, surgical or critical care experience.
• Excellent communication and interpersonal skills.
• Strong clinical assessment skills.
• Ability to manage time with attention to detail and follow-through.

Position Profile

Hospice of the Valley's Arizona Supportive Care (AZSC) team provides support and oversight to seriously ill patients struggling with daily living and disease management. Supportivecare focuses on the patient's physical, psychological, and spiritual needs. The goal is to achieve the best quality of life available by relieving suffering and controlling pain and symptoms. The AZSC RN Case Managerworks closely with patients' physicians and provides case coordination and education. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.

Minimum Qualifications

• Graduate of accredited school of nursing.
• OASIS C experience.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.

• Excellent communication and interpersonal skills.
• Solution driven, creative and resourceful problem solving skills.
• Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.

Preferred Qualifications

• Previous hospice or oncology experience.
• Knowledge of the death/dying and bereavement process.
• CHPN certification.
