Job summary

Comfort Keepers is a licensed home care agency in Oklahoma seeking an experienced Registered Nurse (RN) Case Manager. You will conduct client assessments, develop care plans, perform supervisory visits, and support caregiver training while traveling to client homes in a flexible, part-time/PRN capacity.

Qualifications

• Active and unrestricted RN license.
• Minimum of three years of experience in Home Health, Hospice, Rehabilitation, Home Care or PDN.
• Valid driver’s license and reliable transportation.
• Ability to travel to client homes across the service area.
• Strong computer skills and electronic documentation.

Responsibilities

• Conduct intake calls and initial in-home assessments.
• Develop, implement, and document individualized care plans.
• Perform supervisory visits and supervise caregiver performance.
• Coordinate care among clients, families, physicians, and staff.
• Maintain accurate electronic client records (WellSky) and complete visit documentation.
