Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate, patient-centered care in the community. You will assess, plan and coordinate care for patients in their final days, collaborating closely with families and the interdisciplinary team.

The role includes a company car with fuel and insurance, a competitive salary of $75,000–$90,000 per year, and a benefits package with health/dental/vision, generous PTO, and 401(k) matching.

Qualifications

• RN license required in the state you practice.
• Proficiency with Electronic Medical Records (EMR) systems.
• Strong planning, organizing, and communication skills.
• Ability to work autonomously while meeting responsibilities.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families and healthcare team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
