Job summary

Moments Hospice is seeking an RN Case Manager to join our team on the road, delivering compassionate care across cases and coordinating patient plans. You will conduct initial assessments, provide hands-on nursing care, educate patients and families, and maintain open communication with the healthcare team.

Qualifications

• Licensed as a Registered Nurse (RN) in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while meeting responsibilities.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
