Job summary

Jobot is seeking a hospital-based RN Case Manager for a full-time role in the Hudson Valley NY area. The position follows a Monday–Friday 8:30 am to 5:00 pm schedule with rotating weekend coverage, coordinating care from pre-hospital to post-discharge and managing an assigned unit caseload.

The RN Case Manager will assess, plan and monitor options to create individualized care plans across the continuum, using clinical knowledge, communication, problem solving and time management to optimize

Qualifications

• 3–5 years clinical medical-surgical, ICU or ER experience preferred.
• Managed care experience with utilization review and discharge planning preferred.
• CCM certification preferred.
• Current NYS RN license required.

Responsibilities

• Assess and plan patient care across the continuum of care.
• Collaborate to implement and monitor individualized care plans.
• Use independent judgment to reassess the continuum of care.
• Enforce hospital policies and ensure timely, appropriate clinical care.
