Job summary

Michigan Medicine in Ann Arbor is seeking four Temporary Float RN positions in the Central Staffing Resource Department (CSR) and Care Management. Base wage is $55.00/hr, with Evening and Night Differentials of $5.00 and $3.00 per hour, and a Weekend differential of $5.00 per hour.

The RN Case Manager assesses, coordinates, and monitors a comprehensive care plan in collaboration with physicians, social workers and the interdisciplinary team in inpatient and ED settings; focuses on efficient,

Qualifications

• Current Michigan RN licensure
• Two or more years recent Care Management experience
• Current AHA BLS provider certification
• Bachelor's Degree in Nursing; Master's Degree preferred (or higher)

Responsibilities

• Interview patient/family within 24 hours of admission.
• Lead daily care coordination rounds to update the plan.
• Initiate discharge planning with referrals to homecare, DME and infusion services.
• Collaborate with social work and interdisciplinary team members.
• Ensure patient handovers and documentation in care management software.
