Job summary

Trinity Health Hospital in Grand Rapids, MI is seeking a Float RN Case Manager to support multiple acute care areas including ED, psych/med, med/surg and ICU. This full-time role is on day shift, 40 hours per week with some weekends and occasional holidays.

The position focuses on discharge planning and utilization management to ensure high-quality, cost-effective care. Ideal candidates hold a BSN or ADN with BSN planned within 4 years, a MI RN license, and CM certification.

Qualifications

• BSN or ADN with BSN to be obtained within 4 years of hire.
• Current RN license in Michigan.
• Certification in Case Management or related certification preferred.
• At least 2 years of Case Management experience in an acute care hospital.
• Preferred: 5+ years of Case Management experience.

Responsibilities

• Provide a competent patient assessment by obtaining and interpreting data, making appropriate decisions based on knowledge and experience and adapting plans accordingly.
• Assess patient needs and plan for discharge using evidence-based practice, prioritize and complete comprehensive assessments and facilitate care coordination.
• Manage all cases per established organizational standards and coordinate services across the continuum.
• Monitor and modify a plan of care through interdisciplinary and collaborative processes with the health care team, patient, and family.
• Engage in active listening and empathetic communication with patients, families, and staff.
• Coordinate with patients, families, multi-disciplinary staff, and the community in care planning, delivery and discharge.
• Assess for appropriate resource utilization.
• Establish collaborative relationships with physicians and other health care team members.
• Develop collegial relationships with external customers.
• Serve as patient/family advocate and participate in family meetings and decisions.
• Educate patients and families.
