Qualifications

• Graduation from an accredited Registered Nursing program.
• Current California RN license.
• Current BLS certification.
• Minimum Of Two Years Clinical Nursing Experience.
• Knowledge of Microsoft Office programs.

Responsibilities

• Assess, diagnose, plan, implement, and evaluate patients enrolled in complex case management to coordinate the plan of care with the provider.
• Attend patient appointments in the Priority Care Clinic on site and carry out provider orders.
• Provide patient education and resources to prevent avoidable ER utilization.
• Maintain care plans, document in NextGen, and follow NCQA, CMS, Health Plan guidelines.
• Coordinate social services and resources to support patients and families.
