Job summary

Universal Healthcare MSO LLC is seeking an RN Case Manager to provide advanced case management across the Population Health Management continuum in Bakersfield, onsite. The role covers low-risk outreach to highly complex medical, behavioral health, and social challenges, collaborating with an interdisciplinary care team.

The RN will lead ICP development, monitor interventions, and ensure regulatory compliance while potentially serving at a designated clinic site.

Qualifications

• Current, unrestricted RN license in California.
• BSN preferred.
• Valid driver’s license and reliable transportation.
• Minimum of 2 years nursing experience; case management preference.
• Experience with Medi-Cal/Medicare and/or D-SNP populations preferred.

Responsibilities

• Manage a caseload across the PHM continuum, including high-risk cases.
• Conduct comprehensive nursing assessments of medical, behavioral, and social needs.
• Develop and sign ICPs with members and care teams.
• Reassess members based on risk and plan goals.
• Review care plans with LVNs for accuracy and completeness.
• Provide telephonic, virtual, and in-person outreach to members.
• Assist with scheduling, reminders, transportation, and referrals.
• Educate, coach, and motivate for self-management and adherence.
• Coordinate with providers for safe care transitions.
