Job summary

Qualifications

• Graduate of an accredited School of Nursing; Mississippi RN license required.
• Two years of nursing experience in ambulatory or acute care relevant to Case Management.
• BCLS certification upon hire and/or transfer; TB certification preferred.
• De-escalation training must be completed within 90 days.

Responsibilities

• Coordinate patient care for a diverse age population within the Singing River Health System.
• Evaluate Medicaid client needs, build personalized care plans, and coordinate home/community services.
• Participate in daily discharge planning meetings to address barriers and ensure smooth transitions between care levels.
• Create and maintain communication between inpatient care management and outpatient case management teams.
