Qualifications

• Two (2) years of nursing experience in ambulatory or acute care relevant to Case Management.
• Strong written and verbal communication skills and ability to coordinate patient care.

Responsibilities

• Coordinate patient care across ages within the Singing River Health System.
• Participate in daily discharge planning meetings and address barriers to discharge.
• Create seamless communication between inpatient care management and outpatient case management.
