Job summary

Singing River Health System is seeking a dedicated Case Manager to coordinate patient care and optimize clinical quality and resources within our Pascagoula hospital. You will participate in discharge planning, liaise between inpatient and outpatient teams, and ensure care plans meet policy and regulatory standards.

The role requires nursing licensure in Mississippi, current BCLS, and experience in case management or related nursing fields.

Qualifications

• Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Must be licensed as a Registered Nurse in Mississippi.
• BCLS certification required; TB certification preferred.
• Two years’ nursing experience in ambulatory or acute care relevant to Case Management.
• Strong written and verbal communication, analytical and organizational skills.

Responsibilities

• Coordinate patient care to a population of diverse ages within Singing River Health System.
• Participate in daily discharge planning meetings to address barriers to discharge.
• Create seamless communication between inpatient care management and outpatient case management.
• Evaluate Medicaid client needs and coordinate home and community-based services when applicable.
