Job summary

Singing River Health System is seeking an ER Case Manager for the Ocean Springs Hospital. The role involves coordinating patient care for diverse ages in the ER, with shifts of 10–12 hours and every other weekend, starting at 7am.

Qualifications

• RN with Mississippi license; BSN preferred.
• Two years of nursing experience in ambulatory or acute care relevant to case management.
• BCLS certification and TB testing; de-escalation training within 90 days.

Responsibilities

• Coordinate patient care for a diverse age population within SRHS.
• Attend daily discharge planning meetings to remove discharge barriers.
• Ensure duties follow SRHS procedures, accrediting guidance, and governing policies.
• Assist Medicaid patients with needs assessment and care planning; coordinate home/community services.
