Geisinger Health Plan is seeking an RN Case Manager to coordinate patient care and work with a multidisciplinary team to support employer-focused population health initiatives.

Requirements include at least 2 years of RN experience with case management, Pennsylvania RN license, and knowledge of healthcare laws and IT systems. BSN and ACMA certification are preferred for this role. This full-time position is day-shift and exempt.
