Job summary

Hospice of NW Ohio in Perrysburg, OH is seeking a registered nurse to join our home health team. This role offers a 6th weekend rotation, with only 2–3 holidays per year, and no weekly on-call requirement.

Responsibilities include independent patient assessments, care planning, and collaboration with an interdisciplinary team. Requires Ohio RN license, CPR, and ability to obtain Michigan license if needed; driving is required.

Qualifications

• Graduate of an accredited nursing program with RN license.
• BSN preferred but not required.
• Must hold Ohio RN license and be able to obtain Michigan license.

Qualification Requirements

• Graduate of an accredited school of registered nursing; BSN preferred.
• Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan.
• Minimum 2 years of medical-surgical nursing experience preferred; previous experience in homecare, and/or hospice experience is preferred.
• Has working experience charting in an EMR system, Netsmart system preferred.
• Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
• Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
• Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
• Has the ability to work independently and within an interdisciplinary team.
• Demonstrates strong organizational, time management, and prioritization skills.
• Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
• Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
• Meets state requirements for maintaining professional license.
• Must be CPR certified according to the American Heart Association CPR standards.
