Job summary

Hospice of Northwest Ohio is seeking a qualified RN to join our home care team in Ohio. The RN will provide compassionate, patient-centered nursing care, manage pain and symptoms, and educate families in a home setting.

The role requires strong clinical judgment, ability to work independently and collaborate with an interdisciplinary team. You will work on a schedule that includes every 6th weekend and 2-3 holidays per year, with PTO, a 403b match, and scholarship opportunities; no weekly

Qualifications

• Registered Nurse licensed in Ohio; BSN preferred.
• Home care/hospice nursing experience preferred.
• Ability to communicate effectively with patients, families, and team members.
• Experience charting in an EMR system (Netsmart preferred).

Responsibilities

• Provide basic and skilled nursing care to hospice patients.
• Assess and prioritize patient needs and adjust plans of care.
• Educate patients and families on care plans, pain/symptom management.
• Collaborate with interdisciplinary team to ensure high-quality care.

Qualification Requirements

• Graduate of an accredited school of registered nursing; BSN preferred.
• Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan.
• Minimum 2 years of medical-surgical nursing experience preferred; previous experience in homecare, and/or hospice experience is preferred.
• Has working experience charting in an EMR system, Netsmart system preferred.
• Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
• Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
• Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
• Has the ability to work independently and within an interdisciplinary team.
• Demonstrates strong organizational, time management, and prioritization skills.
• Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
• Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
• Meets state requirements for maintaining professional license.
• Must be CPR certified according to the American Heart Association CPR standards.
