Job summary

Valley Health System seeks a registered nurse with 3+ years of varied clinical experience to lead discharge planning and utilization review. The role emphasizes collaboration with patients, families, and the healthcare team to achieve quality outcomes and maximize reimbursement for patient visits.

Qualified candidates will hold Nevada RN licensure and bring knowledge of social and physical factors affecting discharge and community resources.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum three years of varied clinical experience; two years in Utilization Review, Utilization Management or Case Management preferred.
• Knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
• Proficient computer skills: word processing, spreadsheets, and data management.

Responsibilities

• Establish safe, individualized discharge plans and ensure timely coordination for patient care.
• Provide proficient utilization management to maximize reimbursement for all patient visits.
• Collaborate with patients, families and the healthcare team to achieve quality healthcare outcomes.
• Maintain awareness of patient age-appropriate care requirements.
