Qualifications

• Graduate of an accredited School of Nursing; BS in Nursing preferred.
• Current Registered Nurse License in New York State.
• Strong clinical skills with a solid understanding of acute care hospital policies and processes.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with multidisciplinary teams.

Responsibilities

• Actively manages patient progression from Admission to Discharge.
• Serves as liaison between emergency services, surgical services, nursing, physicians, utilization review and medical records.
• Coordinates safe and appropriate discharge plans for patients within the service area.
