Tallahassee Memorial HealthCare is seeking a qualified RN to perform MDS, case management, utilization review, QA and discharge planning for a 22-county region. You will conduct CMS-based assessments, coordinate discharges, and ensure regulatory compliance in collaboration with the health care team.

Required Florida RN license with at least 1 year of relevant experience; a BSN and MDS certification within 1 year are preferred. This is an equal opportunity employer with a drug-free workplace.

Qualifications

• Associate degree in nursing.
• Florida RN license.
• 1 year of relevant work experience.
• MDS, case management, QA and discharge planning experience preferred.
• Bachelor’s degree in nursing preferred.
• MDS certification within 1 year of employment preferred.

Responsabilités

• Perform Minimum Data Set (MDS), case management, utilization review, QA and discharge planning duties.
• Conduct CMS-based patient assessments, care planning and AHCA documentation.
• Promote wellness and collect data for CMS submission.
• Determine patient needs based on illness, prognosis and social support.
• Coordinate discharges to community or long-term care settings.
• Ensure facility compliance with state and federal regulations; collaborate with the health care team.
