Qualifications

• OR RN license required.

• 2 years recent case management experience in an acute hospital setting.
• Inpatient rehab facility and Med/Surg experience preferred.
• Epic experience preferred.

Responsibilities

• Coordinate interdisciplinary care and discharge planning.
• Assess patients' medical and psychosocial needs.
• Facilitate insurance authorization and utilization review.
• Educate patients and families; ensure safe transition to next level of care.
