Job summary

SSM Health Saint Louis University Hospital is seeking a discharge planning nurse to assess, develop, implement, and evaluate patients' discharge needs in collaboration with the interdisciplinary team.

The role focuses on coordinating with post-acute providers, identifying barriers to timely discharge, and ensuring adherence to CoP discharge planning while supporting readmission reduction efforts. Weekend hours apply with a 24-hour weekly schedule.

Qualifications

• RN licensure required in Missouri or a compact state.
• Graduate of an accredited nursing program.
• Minimum 3 years of acute care experience.
• Weekend shift availability.

Responsibilities

• Ensures compliance with CoP discharge planning.
• Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
• Assesses patients at high risk for readmission and develops a plan with the team.
• Participates with team on opportunities for improvement in standard work.
• May handle utilization management, ED case management, admission processes, and liaise with post-acute providers.
• Applies evidence-based practice and ANA ethical standards to guide nursing care.
