Qualifications

• Graduate of an accredited school of nursing with active RN license in Arkansas.
• Minimum two (2) years of acute hospital clinical experience.
• Masters in Nursing or Case Management field; with relevant experience.
• Basic Life Support (BLS) / CPR within 90 days.

Responsibilities

• Completes and documents discharge planning assessments.
• Leads multidisciplinary discharge planning with patient and post-acute providers.
• Oversees and evaluates discharge plan implementation.
• Collaborates with healthcare team to ensure appropriate inpatient resource use.
• Manages transitions to post-acute services based on clinical needs.
• Identifies readmission risk and implements 30-day interventions.
