Qualifications

• Demonstrates flexibility/adaptability to changing priorities and regulations.
• Basic computer skills.
• Ability to read and communicate in English; bilingual abilities preferred.

Responsibilities

• Conduct comprehensive patient assessments and discharge planning.
• Develop and implement individualized care plans with the care team.
• Monitor patient progress and adjust plans for timely discharge.
• Lead coordination with programs to optimize resource utilization.
• Coordinate care conferences and interdisciplinary rounds.
• Provide education to patients and families regarding discharge and post-hospital care.
• Direct and manage caseload care planning and follow-up.
