Job summary

Sunrise Hospital and Medical Center in Las Vegas, NV is seeking a Registered Nurse Case Manager to coordinate care, manage length of stay, and ensure safe discharge planning in collaboration with the multidisciplinary team.

You will review admissions for medical necessity, coordinate with payors and hospital staff, and facilitate referrals to Home Health, DME, and community resources; we value teamwork and patient-centered care.

Qualifications

• Graduate of an accredited School of Nursing program.
• Current licensure as a Registered Nurse in the state of Nevada.
• Current Basic Cardiac Life Support (BLS) certification.
• Two years in hospital case management; three years nursing experience preferred.
• Knowledge of CMS Rules and Regulations (including Patient Status and Level of Care).
• Ability to maintain documentation directed by the Case Management Plan.

Responsibilities

• Perform admission reviews to determine appropriateness and medical necessity and provide payor reviews as requested.
• Maintain system for monitoring all admissions and ensure timely concurrent reviews.
• Make referrals related to medical necessity, utilization, quality, or risk management and coordinate on-going care with hospital staff, physicians, and services.
• Plan, coordinate, and facilitate all discharge needs for patients including Home Health, DME, placement, community resources, and hospice care.

Education

Graduate of an accredited School of Nursing program

Current licensure as a Registered Nurse in the state of Nevada
