Job summary

Kaiser Permanente seeks an experienced registered nurse to collaborate with physicians to coordinate admissions and discharge planning. You will screen appropriateness of hospital stays, recommend alternative care levels, and liaise with families and community resources to ensure safe, cost-effective care.

Responsibilities include bed management, utilization review, and documenting care plans across settings, while adhering to regulatory standards and patient safety guidelines.

Qualifications

• Two years of RN experience in acute care or case management is required.
• Completion of an accredited RN program that enables licensure as a Registered Nurse.
• Current California RN license and Basic Life Support are required.

Responsibilities

• Coordinate and screen admissions and continued stays with an MD.
• Recommend alternate levels of care when medical necessity for inpatient care is not met.
• Develop and monitor discharge plans and communicate with physicians, families and community resources.
• Participate in bed huddles, utilization management, transfer coordination and outside service authorizations.
• Educate patients and families about discharge processes and post-hospital needs.
