Qualifications

• RN program completion required by date of hire.

Responsibilities

• Coordinates post-discharge patient care to ensure timely and effective discharge.
• Performs and documents thorough, age-appropriate patient assessments.
• Develops mutually agreeable plans of care with patients and families.
• Facilitates safe transfers and coordination with care teams.
• Ensures regulatory and compliance standards are met in care planning.
• Maintains ongoing communication with interdisciplinary staff.
