Job summary

Kaiser Permanente is seeking an Inpatient Care Manager (RN) to coordinate discharge planning and transition care for acute and emergency settings. You will assess patient needs, develop care plans, and work with physicians and the care team to optimize resources and quality of care.

Qualified candidates will have at least two years of RN experience in Med/Surg, ICU, ED, or related areas, and a strong ability to advocate for patients while ensuring regulatory compliance and accurate documentation.

Qualifications

• Minimum of two years RN experience in Med/Surg (hospital acute care), ICU, Emergency Department, Home Health, Skilled Nursing Facilities, Hospice or related areas.
• RN License (WA) within 6 months of hire OR Compact License: RN within 6 months of hire; RN License (Oregon) within 6 months of hire.
• Basic Life Support (BLS) within 3 months of hire.
• Demonstrated ability to interrelate with physicians, nurses, and patients in an interdisciplinary approach.
• Ability to develop concise and thorough documentation of patient clinical assessment and care needs.

Responsibilities

• Coordinates post-discharge patient care needs to assure timely and effective discharge from the hospital.
• Independently completes and documents patient assessments that are thorough, timely, age-appropriate, and reflect psychosocial support.
• Coordinates with patients, families, and health care team to develop mutually agreeable plans of care.
• Facilitates resolution of issues to ensure safe transfers and timely, appropriate care with improved utilization of resources.
• Maintains ongoing contact with interdisciplinary staff and continuing care services for safe transitions across care settings.
• Ensures regulatory and compliance standards are met in collaboration with the health care team.
• Performs duties as requested.
