Job summary

Qualifications

• Minimum of five (5) years’ experience in a clinical nursing setting or case management.
• Supervisory experience, preferred.
• RN Registered Nurse and BLS required.
• Knowledge of HIPAA, quality improvement, and care coordination.

Responsibilities

• Assist the Director with development and oversight of the case management program.
• Oversee case management staff, including training and performance management.
• Coordinate care with healthcare providers and community resources to ensure seamless patient care.
• Provide regular KPI reporting, monitor outcomes and program compliance.
• Represent the program to leadership and external stakeholders.
