Job summary

Southern Tennessee Regional Health System - Pulaski is seeking an experienced Registered Nurse Case Manager for a full-time, day-shift role. You will consult with the nursing staff and the multidisciplinary team to evaluate patient status, coordinate discharge planning, and ensure timely progression of care.

Responsibilities include chart reviews, addressing patient and payor questions, and participating in performance improvement.

Qualifications

• Requires critical thinking, decisive judgment, and ability to work with minimal supervision.
• Experience in Utilization Review, Quality Improvement and Case Management is preferred.
• Active Tennessee RN license (compact license acceptable).
• Graduate from an accredited school of nursing; BSN preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness.
• Monitors patient and family satisfaction and addresses questions from patients, families, and payors.
• Leads discharge planning and coordinates patient transfers to other facilities; provides discharge education.
• Performs chart reviews focusing on service delivery, outcomes, cost, and reimbursement.
• Communicates with providers, payors, and ancillary services to remove barriers to timely care progression.
• Documents case management activities, plans and outcomes accurately.
