Job summary

Southern Tennessee Regional Health System - Pulaski is seeking a Case Manager RN to join our team. You will coordinate discharge planning, collaborate with nursing staff and the multidisciplinary team, and ensure timely, appropriate care while supporting patients and families.

Requirements include an active Tennessee RN license and an associate's degree in nursing (BSN preferred). Certification in BLS within 30 days is expected.

Qualifications

• Active Tennessee RN license required.
• Associate's degree in nursing required; BSN preferred.
• BLS certification to be obtained within 30 days of hire.
• Two years of related clinical experience in Utilization Review, Quality Improvement and Case Management preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status, admission, length of stay, transfers and discharge.
• Monitors patient and family satisfaction; addresses questions and payer concerns.
• Coordinates discharge planning and referrals to community resources; provides discharge education.
• Performs chart reviews to improve service delivery, patient outcomes, and satisfaction; addresses cost and reimbursement issues.
• Communicates with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Documents case management activities, plans, and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations and may work alternate shifts or be on-call.
