Job summary

Coast Plaza Doctors Hospital is seeking a Nurse Case Manager to oversee clinical resource utilization and documentation affecting reimbursement. You will monitor patients, assess medical, financial, psychosocial, and physical needs for discharge planning, and coordinate with staff to ensure efficient transitions.

Qualified applicants should have at least two years of RN or Case Manager experience, strong communication, and collaborative skills to support a mission-driven team.

Qualifications

• Two years’ experience as a RN or Case Manager required.
• Critical thinking, service excellence and good interpersonal communication skills.
• Ability to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with team leaders.

Responsibilities

• Review daily clinical information to determine discharge readiness with physician and team.
• Coordinate with the Discharge Planning Assistant to arrange post-acute care.
• Attend HPIRounds to remove discharge barriers and manage LOS.
• Initiate discharge planning after social work interviews and collaborate with care teams to create safe transitions.
• Communicate patient status and LOS in huddles and rounds; document changes in the EHR using SBAR.
