Job summary

Houston Methodist Willowbrook Hospital is seeking a PRN Case Manager RN to plan and coordinate case management for a designated patient population on a unit. The role collaborates with physicians and the health care team to ensure medically necessary admissions and efficient discharge planning.

The CM PRN will work with social workers to ensure continuity of care, communicate with payors for reimbursement, and contribute to performance improvement through evidence-based practices.

Qualifications

• Three (3) years hospital clinical nursing experience including two (2) years in case management.
• RN - Texas State Licensure must be obtained within 60 days if establishing Texas residency.

Responsibilities

• Collaborates with physicians and the interprofessional health care team to facilitate care for designated case load and ensures plan of care is patient-focused, high quality, and cost-effective.
• Precepts and coaches staff; serves as a resource for case management and social work resources.
• Reviews medical necessity for admissions and continued stay; applies utilization criteria to monitor appropriateness and communicates with payors as needed.
• Documents assessment and interventions; plans for discharge and manages post-discharge processes to ensure continuity of care.
• Provides education on case management topics and pursues practice improvement through evidence-based initiatives.
