Job summary

HonorHealth is seeking an RN Case Manager at Shea Medical Center in Scottsdale, AZ. This full-time, in-person role supports care coordination for high-risk acute care patients and manages discharge planning for complex post-acute needs.

Requires an RN license and 3 years hospital experience; 5–7 years in case management preferred. Day shift, Monday–Friday, with a sign-on bonus up to $10,000.

Qualifications

• Associates in Nursing required.
• Bachelor's in Nursing or healthcare-related field preferred.
• 3 years clinical hospital experience required.
• 5 years RN in Case Management preferred.
• 7 years RN experience preferred.

Responsibilities

• Guides care coordination for high-risk acute care patients.
• Facilitates discharge planning for medically complex post-acute needs.
• Analyzes medical records for admission validity, treatment, and LOS.
• Advocates for patients/families and liaises with care team.
• Coordinates transitions of care with stakeholders to ensure timely services.
