Shifts are day time with 8-hour days, starting 10/26/2026. This onsite position at Mission Hospital offers a structured 13-week assignment with Cerner, NATE, and Ensocare workflows in post-acute care contexts.

• Strong documentation skills.

Responsabilidades

• Coordinate case management activities across hospital units.
• Maintain accurate and timely documentation in Cerner, NATE and Ensocare.
• Communicate clearly with patients, families and interdisciplinary teams.
• Support self-scheduling and timely patient care planning.
• Collaborate with RN Case Managers to ensure optimal patient outcomes.
